NOTE: After you fill this form out and press submit, a copy will be emailed back to you. Sign the copy and submit it with your entry fee. Pressing submit does not enroll you into the tournament. It merely notifies our organizers of your desire to play so that they can add your numbers to the list. You must sign and mail in the form along with the entry fee by July 25, 2011 in order to be entered into the tournament. Teams based within 80 miles of the field sites may be required to play on Friday evening. I, the coach or manager of the above named team assure that all information is correct and also understand that if my team is not accepted, the entry fee will be returned in full. I further understand that once my team is accepted and later withdraws, the entry fee is forfeited. No refund will be made in the event of cancellation or shortening of any matches due to inclement weather. I agree to abide by all the U.S.Y.S.A., M.S.Y.S.A. and Tournament Rules. I understand that if my team usually plays small sided games, that the Dearborn Soccer Invitational Committee may require it to play a larger sided game as stated in the Tournament Invitation. I have read the Tournament Invitation and will abide by its rules.
